For Individual Registration:

This method of registration is for those participants wanting to join the league, do not have a team and would like to be placed on team. All players, other than goaltenders, will be guaranteed to be placed on a team within the league.

Costs: Guaranteed 26 games, $480

For Team Registration:

A team has a maxium of 16 players (15 skaters and one goalie).

Costs: Guaranteed 26 games, $7000

1. The Team Rep must first create an account with QuickEnrollment "Security Profile"
   
2. Team Rep would register herself, follow the prompts all the way to the payment page where you select cheque as your form of payment, follow prompts to finish registration.
   
3. Team Rep would then pass along the id/psswd they created, to their team mates
   
4. Each player on the team would then use the id/psswd to logon
   
5. Once you are logged in to the Team Rep's, "security profile"
   
   • you would click "no" to the question "do all the participants you want to registar on the list?"
   • you would add "your own" information to the main contacts profile and click submitt
     
   • you would then click "yes" to the question "do all particpants...".  
   • Follow the prompts all the way to the payment page where you will select cheque as your form of payment.
   • Follow prompts to finish registration
     
6. Team registration fee due August 26, 2011,One cheque of $7000 is payable to "Newtonbrook Women's Hockey Club". Please print your name and team on the cheque. Mail the cheque and this Team Registration form to:

For Your Reference - Hockey Skill Level:

1. (Beginner) never played before; not a strong skater; can stop
2. (Still Learning) played in a recreational league, have taken some lessons, skate fairly well, not a strong stick handler, understand the rules of the game
3. (Intermediate) played recreational tournaments, can skate well, can stick handle, understand the rules of the game
   
4. (Advanced) Currently play/have played at a C competitive/organized level AND/OR skates very well, knows positional play, can stick handle
   
5. (Olympic Hopeful) currently play/have played at a AA, A, B competitive/organized level AND/OR a very strong skater & excellent hockey skills
